To comply with the newly effective House Bill 1284 and Senate Bill 109, the Colorado Department of Public Health and Environment has released new patient forms. Old paperwork will still be accepted through July 9 — as well as anything already submitted, of course — but will be returned back to the applicant afterward.
New to the patient form is a requirement to identify the source of the medical marijuana (self, caregiver, MMC) and if it's a center, the name and address.
New to the physician authorization form is the date of the patient's exam, and confirmation of the physician's authorization by the Drug Enforcement Agency to recommend controlled substances for treatment. Lastly, if the diagnosis includes "severe pain," the form must include the reason.
